When a Breast Lift?

There are several reasons that cause a breast to sag and hang. Ptosis or sagging can occur due to pregnancy, weight loss, or genetic predisposition. Normally, the nipple projects itself in the middle of the breast, the volume of the upper pole is about the same as the volume of the lower pole.

 We distinguish:

  • Grade I Slackening: When the nipple is still above or at the level of the fold under the breast.
  • Grade II Slackening: Is a more extensive form of slackening in which the nipple projects below the fold.

In both cases, the upper part of the breasts often has no filling left. 

With a Grade I slackening a nipple lift can be obtained by a breast enlargement with a larger implant (e.g. 290 cc to 350 cc).  

With Grade II slackening, a nipple lift can only be performed by lifting, usually in combination with breast augmentation to restore fullness to your breasts. Different incisions are possible.

The steps of a Breast Lift

During consultation, the doctor will determine whether you are a suitable candidate for a breast lift. Everything related to the treatment will be discussed such as: what method is best for you, what results to expect, the possible risks and the aftercare.

The doctor will ask you about your medical history, what medications you are currently taking and so forth. Note that if you use any blood thinning medication, you should stop at least one week before the treatment. The doctor will also ask what your wishes and expectations are. He can then indicate whether the surgery can meet those expectations and whether a combination with another treatment method (e.g. breast augmentation) can be an option. 

In addition, it will be discussed which breast lift technique will be performed in your case. The different techniques include: the donut lift- an incision around the areola (with a small elevation of a maximum of two centimeters), the lollipop lift- an incision around the areola and straight down to the fold of the breast (in case of moderate sagging or flaccidity) or an anchor incision- where an incision is made around the areola, straight down to the fold of the breast and horizontally along the breast crease (this is done when your breasts are heavily sagging). The excess skin is removed and the nipple is moved upward. If necessary, the shape of the nipple is also corrected.

Before the treatment, pictures are taken. Then the plastic surgeon marks out where the incisions will be made and where the nipple will be placed. When implants are placed, the surgeon will first do the breast lift, then place the implants and finally close the incision in the inframammary fold. After the surgery, you will receive a bandage that supports and protects your breasts. The procedure takes about one and a half to two hours. The surgery is performed under the insertion of a local anesthetic fluid into the breast (local anesthesia) in combination with sedation or general anesthesia, which will allow you to recover more quickly and allow the procedure to be performed on an outpatient basis.

When you feel well and the doctor is satisfied, you will be discharged. You are not allowed to drive, therefore ensure that you arrange for someone to drive you home. During the first two weeks, you will have to wear a sports bra- day and night (after four weeks you only wear it during the day) – that supports the new shape of your breasts and prevents swelling. On average, the swelling will be significantly less after three days. After 48 hours, you can shower again and take off the bra.  We recommend that you rest for a few days after the operation. You can then resume your daily activities, but avoid heavy exertion during the first weeks. It is important that during this time, there is no pressure or tension on your breast. Two weeks after the treatment you will return to the clinic to have the stitches removed. The swelling will disappear in the course of two to six weeks. In the beginning, the feeling in your nipples may be different. This varies from very sensitive to numb. You will gradually have the normal feeling in your nipples. It is difficult to tell how long it will last as it varies from person to person. The first months, the scars can be quite fiery. After that they will steadily take on the color of the skin and become less visible. It is important not to expose the scars to the sun during the first year after surgery.

In experienced hands, breast augmentation and/or lifting is a low risk procedure.

Sensory disorders (usually temporary and rarely permanent).

Hypertrophic (= strongly thickened) scars or delayed wound healing.

Encapsulation (< 1%);

Infection (1-1.3%).
